About MyPlace Health
myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.
Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.
myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.
About This Role
Are you a mission-driven leader who thrives on making a real difference in the lives of older adults? We're looking for a compassionate, organized, and hands-on Transportation Coordinator to oversee all transportation services and personnel in our PACE program. Based onsite at our Compton, CA Center, this full-time role (Monday through Friday, 8:00 AM to 5:00 PM) plays a vital part in ensuring our participants experience safe, timely, and dignified transportation every day.
As the Transportation Coordinator, you’ll serve as a key member of the Interdisciplinary Team (IDT), helping shape the plan of care for each participant. You’ll coordinate with drivers and fellow team members to ensure the seamless delivery of meals, medications, equipment, and other essentials—going beyond just rides to create meaningful connections and dependable support.
This is a fantastic opportunity for someone who not only has experience leading transportation operations in a PACE or similar healthcare setting, but who also brings a spirit of continuous improvement and a heart for service. You’ll have the chance to grow with a collaborative team, lead with purpose, and directly impact the quality of care our participants receive.

- Follow the rules and stay detailed: Ensure compliance with all PACE transportation regulations and maintain accurate records, including transportation logs, fuel card usage, and vehicle maintenance.
- Pitch in behind the wheel: Step in to drive participants when needed—to and from the center, appointments, or home—ensuring a safe and positive experience.
- Continuously improve: Create and refine workflows that deliver a high-quality, safe, and participant-centered transportation experience.
- Coordinate with care: Serve as a key liaison between external vendors and the transportation team to keep everything aligned and efficient.
- Prioritize safety: Perform regular safety checks on all vehicles and work closely with drivers to address and resolve any concerns promptly.
- Pitch in where needed: Take on additional duties as assigned to support the team and ensure participants receive the highest quality of care and service.
